After more than seven decades of dedicated service, we sadly announce the closure of Wahiawa General Hospital. Since our founding in 1944, we have been honored to serve the healthcare needs of the Central Oahu and North Shore communities.

We extend our heartfelt gratitude to our staff, physicians, volunteers, and supporters for their unwavering dedication and commitment to compassionate care. Your contributions have made a lasting impact on countless lives.

For All (Medical and Non-Medical) Records Requests:

All record requests must be submitted via email to [email protected].

Authorization for Release of Medical Information

To ensure the confidentiality and security of medical records, all requests for medical information must be submitted via email, to Wahiawa General Hospital. For your convenience, a downloadable and fillable form is available for use.

Please note the following requirements for processing requests:

All requests must be in writing and signed by the patient (18 years or older), legal representative, or guardian. Signatories must provide a valid photo ID.
Kindly allow up to 30 business days for the processing of your request.
You may access the “Authorization to Release Medical Information” form for printing and completion. Once filled out, please return the form via email to [email protected].

We do not impose any charges for record duplication. However, if a patient wishes to have information sent to a physician or healthcare facility at the direction of the healthcare provider, verification of the request is necessary before any records are released.

WGH 403(b) Retirement Contributions (MetLife):

Aloha Past Wahiawa General Hospital Employee,

Thank you so much for your previous service to Wahiawa General Hospital (WGH)!

WGH is now in a position to make past contributions to the 403b MetLife Retirement Plan for years between 2016 and 2024.

If you were an eligible employee of WGH within this time frame, WGH intends to pay these funds directly to you.
